At Reverence Health, spiritual care is an essential part of what we do. In partnership with SpirituWell, we provide compassionate, professional spiritual care through secure telehealth services for clients and their families, meeting them wherever they are with presence, wisdom, and respect.

Our spiritual care services are designed to support individuals as they navigate the deeper questions and challenges that often arise with aging, illness, memory loss, and caregiving. Research consistently demonstrates that unmet spiritual distress can negatively impact emotional health, coping, and overall quality of life, particularly among older adults. By addressing spiritual needs alongside physical and emotional care, we help foster peace, meaning, and resilience during vulnerable seasons of life.

Our Assessment Approach: The GERO-6 Model

Spiritual care at Reverence Health is guided by the GERO-6 Spiritual Assessment, an evidence-informed framework developed specifically for older adults. The GERO-6 identifies six core domains where spiritual distress commonly emerges in later life and chronic illness:

Identity

Questions of purpose, meaning, and self-understanding as roles and abilities change

Legacy

Reflections on one’s life, regrets, accomplishments, and desire for one’s life to matter

Connection

Needs related to relationships, belonging, reconciliation, and intimacy

Autonomy

Concerns about dignity, control, and being heard in decision-making

Death & Dying

Fears, hopes, and questions related to mortality and end-of-life peace

Religious & Spiritual Needs

Relationship with God, the Sacred, or Transcendent, as well as faith practices and beliefs

Through SpirituWell’s telehealth platform, clients and families may receive:
